Another novel assistive reproductive technique, Testicular Sperm Extraction (TESA) and Microepididymal Sperm Aspiration (MESA) are employed, where fertility specialists are very sure that the causes of male fertility can be ascribed with a fair degree of accuracy to the male partner.


Through this technique doctors pass a very fine needle through the scrotum (by numbing the area) into the epididymis and fluid inside is aspirated out. Sperms obtained from this area are usually older but motile (moving).


Another variant of this technique is Testicular Sperm Aspiration (TESA) where a surgical needle is passed into the scrotum and a small amount of testicular tissue is aspirated out. Thereafter, sperm is separated from the testicular issue which is comparatively young.


Sperms obtained from MESA or TESA need to be fertilized in the laboratory using ICSI or IVF procedures and empirical evidence has shown that sperms obtained from epididymis are more motile and easier to identify by embryologists.


These techniques are very useful in cases in cases:


  1. Where there is no sperm in semen (azoospermia) and no direct causeof this complication can be found so it is better to aspirate sperms surgically.
  2. Where there is backward ejaculation into the bladder called as retrograde ejaculation.
  3. Where there is obstructive azoospermia owing to an obstruction in the outflow tract from the testicles to penis.
  4. Where there has been a situation of irreversible vasectomy, absence of vas deferens, diabetes, and quadriplegia.
  5. Where the patient is taking certain anabolic steroids /HGH, prescription medication or alcohol/marijuana which can also cause azoospermia.
